Notes – Reebok San Francisco 49ers Alex Smith #11 jersey, as worn during his rookie season, after being selected as the first overall pick in the 2005 NFL Draft. Smith is a 3-time Pro Bowler, who was the NFL passer rating leader in 2017. During his final season with the 49ers the franchise reached Super Bowl XLVII, however Smith lost the starter spot to Colin Kaepernick, midway through the season.
